Registration forms no later than Aug 15, 2010

I want to be the first to welcome you to FLFS 2010. There is just a few things I would like to go over with you about registration to make the registration process smooth for you.

You will find a registration form that will have classes listed, please indicate using 1, 2, or 3 the preference for the choice of class you would like to take. We have limited space for students, so if there is no space available for you in your first choice we will have an alternate class selection for you. This is a first come first serve registration. Early registration will help you with acquiring your class selection.

Registration fee listed below is per person per day. Two day classes will be one fee for both days. Payment must be submitted to register. You may sign up online, but registration will not be complete until a check is recieved. There will also be State form for each VFA class that we offer at FLFS that needs to be filled out if you take a VFA class. It is mandatory for each student to have a class form filled out and signed by your chief on record before you can take a class. Please return the forms to FLFS and not VFA it makes registration very hard if we don’t know that the form has been filled out. We will mail them to the state for you.

Please read and sign the Waiver, and return it with your check. All forms, and payment are very important in order for us to provide you with the class that you would like to take. Please feel free to call me if you have any questions or concerns at 1-802-793-2090

School Policies

1. Registration

  • Print registration and waiver form no later that August 15, 2010
  • Registration fee must accompany registration form.
  • Registration fees are not refundable.
  • When filling out the registration form, indicate a first, second, or third choice next to the appropriate course on the application form.

2. Certificates will be issued at the close of each class, after receipt of the evaluation form.

3. This school/corporation is not in any way affiliated with any political party or government, in the United States or Canada, whatsoever, and does not endorse or recommend, either factually or implied, any of the vendor items on display or used for instruction in any of its programs.

4. Students must be facial hair compliant for all classes requiring SCBA equipment.

Student Regulations

1. All registrants must be members in good standing of a Recognized fire, or rescue organization.

2. All students attending the school must do so with the consent of the head of their organization, verify they are covered by their organizations insurance and be physically qualified.

3. FULL PROTECTIVE GEAR is a requirement of all students attending a program involving field training exercises.

4. HORSE-PLAY/ALCOHOL/ DRUGS will not be tolerated. Students found to be involved in this type of activity will be expelled from the program and will not be allowed a certificate.

5. It is the student’s responsibility, along with the instructor’s to keep the classroom and field training sites clean.

6. By virtue of Vermont State Law, smoking is NOT PERMITTED on school grounds. To comply, you must be completely off the property, across the street.
